How We Work

We at Daniel Jarvis Home Health understand that having a say in planning your care is important to you. This is why we take a step-by-step approach to our intake process to ensure optimum care and no confusion for our clients and their family. Here is an overview of the process.

1. You or a family member requests our services.
2. The request is sent to the primary care physician for approval.
3. Once approval is received, we add the new client to our services and schedule them for visits.
4. An RN comes to your home to do a Start of Care assessment. This includes a physical assessment of body systems and evaluation of client’s needs; which could include referrals for the following:
   • DME supplies
   • Speech Therapy, Physical Therapy and/or Occupational Therapy
   • Skilled Nursing
   • Medical Social Work
5. Follow-up visits will be scheduled and services will continue.

Daniel Jarvis health care professionals are dedicated to the care and comfort of our patients and are HIPPA compliant. Our patients information is handled in a confidential manner.

Client Relationships

The relationship we have with our clients is a strong one. We are here to help keep you healthy and independent, answer any questions you have, and give you excellent home care! We do monthly customer satisfaction surveys and our nurses and therapists give you time to ask questions and express concerns during every visit. If you have anything that comes up before or after your visits then we have friendly professional office staff that are ready and available to help.

TeleHealth Monitoring

Daniel Jarvis is one of only a handful of home health care agencies which specialize in TeleHealth – a remote patient monitoring system for 24-hour care. Your home health nurse specialist has continual access to monitor your health status via your telephone line, with the use of ultra sophisticated health telemetry units.
